It’s easy to register as a patient at Millbrook Medical Centre

We’re using a new online service called “Register with a GP surgery” to make it simple for you to sign up with our practice.

Just fill out this quick online form to get started. You don’t need proof of address, your immigration status, ID, or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It helps us with less paperwork and makes it easier for you to register. If you prefer, we still have paper forms available.

It can take up to 7 working days to finish processing your registration.

Once we accept your registration, if you’re 16 or older, you’ll be asked to come to the Bewley Drive practice. You’ll need to use the Self Health Kiosk for a New Patient Medical screen.

If you live outside our practice boundary, we can’t accept your registration.
You’ll find out about this when you register, or you can check our map below to see if your postcode is inside our boundary.

If you’re between 40 and 75 years old and you’re one of our patients, you can have a free NHS Health Check. These happen every 5 years and you’ll see one of our Healthcare Assistants for it. If you think it’s time for your check and you want to make an appointment, just ask at Reception.

Temporary Patient Registrations

If you’re away from your usual GP, and you really need to see one, you’ll need to sign up as a temporary patient.

To sign up as a temporary patient, you’ll have to fill out a form called a GMS3 Temporary Registration Form and give it back to the practice.

You can register as a temporary resident with a GP in England if you are in the area for longer than 24 hours, but less than three months.

Find Your NHS Number

Sometimes when filling in online forms or speaking with our staff, you may be asked for your NHS number.

This online tool will help you find your number if you do not know it.

You do not need to know your NHS number to use NHS services, but it can be useful to have it.

Other ways to get your NHS number

If you cannot get your NHS number online you can:

  • find it on any letter from the NHS like a prescription or appointment letter
  • call your GP surgery and ask them for your number
