Travel Vaccinations

Travelling abroad? You may need certain vaccinations

Please fill out the Travel Risk Assessment Form below in plenty of time before your trip (it’s really important to tell us as soon as you can – at least 6 weeks before you go). This helps us figure out what vaccinations you might need for the countries or places you’re visiting.

After you fill out the form, the Practice Nurse will call you, usually within 10 days. You might need a second appointment with the Nurse to actually get the vaccinations after the phone call. Your second appointment should be at least 2 weeks before your trip to give the vaccines time to work.

Some of the vaccines we need for travel have to be ordered, and not all travel vaccinations are included in the services provided by the NHS. If you need vaccines that aren’t covered by us, we might send you to another place to get the remaining vaccinations, such as Liverpool School of Tropical Medicine.

Please complete the form below to get more information about what travel immunisations you require. Most vaccines are given at least 2 weeks before travel, and some more complicated regimes take longer. Please try to give us prior notice (preferably 6 weeks).
